Patient Discharge Policy
At Griffis Grove Health Collective, we are committed to providing safe, respectful, and high-quality care. To maintain an effective provider-patient relationship and ensure appropriate medical care, patients are expected to comply with practice policies, treatment recommendations, and follow-up requirements.
The practice reserves the right to discontinue services and discharge a patient from care when continuation of the provider-patient relationship is no longer appropriate, safe, or feasible.
Missed Appointments and No-Shows
Patients may be subject to discharge if any of the following occur:
Three (3) missed appointments within a rolling twelve (12) month period.
Two (2) consecutive missed appointments without prior notice.
Repeated late cancellations (less than 24 hours' notice) totaling three (3) occurrences within a rolling twelve (12) month period.
Failure to attend required follow-up appointments necessary for safe ongoing treatment.
Failure to Complete Required Monitoring
Patients receiving ongoing treatment may be discharged if they:
Fail to complete required laboratory testing within the timeframe requested by the provider.
Fail to complete required follow-up visits within six (6) months of their last appointment.
Repeatedly fail to respond to outreach attempts regarding required monitoring, abnormal laboratory results, or treatment follow-up.
Request ongoing medication management without completing required clinical monitoring.
Noncompliance with Treatment Requirements
Patients may be discharged for:
Failure to comply with treatment agreements, informed consent documents, or practice policies.
Failure to provide accurate and complete medical information.
Repeated failure to follow reasonable treatment recommendations when such noncompliance creates concerns regarding patient safety.
Financial Responsibility
Patients may be discharged for:
Repeated failure to maintain a valid payment method on file.
Chargebacks, payment disputes, or fraudulent payment activity.
Failure to satisfy outstanding balances after multiple attempts to resolve the matter.
Repeated failure to comply with agreed-upon payment arrangements.
Inappropriate Behavior
Immediate discharge may occur for:
Threatening, abusive, harassing, intimidating, or disruptive behavior toward providers, staff, contractors, or other patients.
Discriminatory, hateful, or sexually inappropriate conduct.
Fraudulent activity, identity misrepresentation, or falsification of medical records.
Attempts to obtain medications, prescriptions, or services through deception or manipulation.
Telehealth and Geographic Limitations
Patients may be discharged if:
They relocate to a state where Griffis Grove Health Collective is not authorized to provide care.
They repeatedly attempt to receive care while physically located in a state where the provider is not licensed.
Their healthcare needs exceed the scope of services offered by the practice.
Medication Management
The practice may discontinue prescribing and/or discharge a patient if:
Required follow-up appointments are not completed.
Required laboratory monitoring is not completed.
The provider determines continued treatment is not medically appropriate.
The patient requests treatment that falls outside evidence-based practice, professional standards, or applicable laws and regulations.
Notice of Discharge
Except in circumstances involving immediate safety concerns, fraud, or threatening behavior, patients will generally receive written notice of discharge.
The notice may include:
Effective date of discharge.
Instructions for obtaining medical records.
Information regarding continuity of care when appropriate.
Medical Records
Patients may request copies of their medical records in accordance with applicable laws and practice policies.
Emergency Care
Griffis Grove Health Collective does not provide emergency medical services. Patients experiencing a medical emergency should call 911 or seek immediate emergency medical care.
Practice Discretion
Nothing in this policy limits the provider's ability to exercise professional judgment regarding patient care, patient safety, continuity of care, or compliance with applicable laws and regulations.
